The CMS Guiding an Improved Dementia Experience (GUIDE) Model

What is GUIDE?

The Centers for Medicare & Medicaid Services (CMS) announced on July 31, 2023, the Guiding an Improved Dementia Experience (GUIDE) Model, a voluntary nationwide alternative payment model focused on comprehensive, coordinated dementia care, aimed to improve the quality of life for individuals with dementia. It reduces the strain on unpaid caregivers, enabling them to remain in their homes and communities. The model achieves these goals through Medicare payments for a comprehensive package of care coordination and management, caregiver education and support, and respite services.

The GUIDE Model has two tracks for healthcare participants: an established program track and a new program track. Established participants began July 1, 2024 and new participants began July 1, 2025. The model will run for eight years.


  • The Alzheimer’s and Dementia Care (ADC) Program aligns perfectly with CMS’s voluntary GUIDE model, and its requirements to improve the support of people living with dementia and their unpaid caregivers.
