  • Initial Interest Form - provides the project team with basic information regarding the point of contact and their organization

  • Initial call with UCLA team - a telephone call with the ADC Project team will be coordinated with the organization. During the call the following will be discussed:

    • Overview of the organization’s current state and plans regarding dementia care

    • The ADC program model

  • OPTIONAL: Lynn Spragens is available to meet with dissemination sites to discuss the business case

  • The project team will send a Readiness Assessment form for completion and materials for review and to share:

    • 1-pager Summary of The ADC Program

    • Technical Assistance Support

    • Minimum Requirements of an Adopting Site

  • The project team will review completed Readiness Assessment forms to determine whether the interested site is:

    • Not yet ready for dissemination; project team will provide ‘homework’

    • Ready; project team will send a Letter of Agreement and contract to for review and signature

  • Begin adoption process
